The case manager is an RN who collaborates with interdisciplinary team members, patients and families to facilitate the transition of the patient from the hospital to a post-hospital setting. The case manager plays an integral part in the utilization and resource management process.
QUALIFICATIONS:
- Licensed in the state of Oklahoma
- Minimum of 2 years clinical experience.
- Case management experience preferred.
- Strong skills in oral and written communications.
- BSN preferred.
PHYSICAL REQUIREMENTS:
- Able to speak clearly and distinctly with staff, physicians, patients and families.
- Able to extensively utilize telephone for communication.
- Demonstrates adequate physical and emotional stamina to deal with stressful situations and to complete the necessary work schedule.
- Able to sit/stand/walk for extended periods of time throughout the work schedule, with some light lifting and bending.
- Demonstrates adequate vision for reading and preparing written documents.
- Able to hear spoken communications from staff, physicians, patients, and families as well as pages.
- Demonstrates adequate manual dexterity to prepare written documents.
- Able to work in a latex burdened environment.
Location: Stillwater Medical · Case Management
Schedule: Full Time, Days, Monday-Friday 8:00am-5:00pm.
