Treatment Coordinator

Location: Ona West Virginia

Provide leadership in partnering with families and appropriate others in the community to assure that the therapeutic needs of a designated caseload of troubled children are! adequately addressed. In each case the Treatment Coordinator will assure that the services delivered both to the target child/adolescent and to the family are individualized, appropriate to need, child centered, family focused, culturally competent, strengths based, and goal driven.

Essential Roles and Responsibilities

1. Supervision, Training and Support
To assure quality and model fidelity the Treatment Coordinator will (1) assist and support in training activities for TFC parents (2) directly supervise all activities of treatment parents related to the treatment and care of TFC youth through regular home visits and phone calls (3) provide in-home training as needed for TFC parents (4) complete a formal TFC Parent Evaluation including creation and monitoring of a professional development plan for each family (5) provide 24-hour emergency “on-call” service to TFC families according to designated schedule (6) assist in the arrangement of respite s! ervices
2. Documentation and Compliance
To assure qual! ity service and compliance with regulatory bodies, the Treatment Coordinator will (1) maintain documentation of in-home and other ongoing training activities (2) prepare written summaries, Individual Service Plans and quarterly updates, discharge summaries, and program contract regarding youth and families (3) ensure completion of and collect TFC parent documentation, including LODES, Medication Logs, Respite logs, copies of medical information, and any other required documentation
3. Treatment Planning and Service Delivery
To assure the development and implementation of educational and treatment plans at school, home, and in the community, the Treatment Coordinator will (1) assess each child’s needs in all life domains and develop measurable and objective treatment plan goals (2) provide ongoing advocacy and support for the child’s educational needs (3) Initiate and coordinate psychiatric, psychological, and other supportive therapeutic services when appropriat! e (4) monitor the progress of the child’s goals and modify the treatment plan as needed (5) provide overall leadership and coordination of the treatment team

4. Safety & Well Being
To assure the child’s safety and well being, the Treatment Coordinator will (1) assure that all policies, procedures and standards relating to child safety and well being are followed (2) make regular contact with each youth to assess safety and well being (3) assure that TFC parents follow procedures for child illness and provision of routine healthcare (4) develop an individual safety plan for each child and assist other staff, in crisis intervention to protect youth, staff, and property (5) when transporting youth, maintain safe driving procedures (6) adhere to mandated child abuse reporting laws
5. Working with Biological Family
To maximize the likelihood of a positive relationship between TFC youth and their biological families, the Treatment Coordinator will (1) ass! ess and identify the needs of the birth family to achieve their family ! service plan goals (2) empower birth families through systems education and advocacy training (3) involve the biological family in all aspects of treatment planning including regular updates on youth’s progress (4) coordinate regular contact between the child and his/her biological family as appropriate
6. Communication & Community Resources
To assure continued communication between the programs, the child’s family, relevant
community agency and case aides, the Treatment Coordinator will (1) develop each youth’s treatment team including formal and informal supports as identified by the youth, family, and guardian (2) maintain regular contact and open lines of communication with youth’s treatment team and any other significant community persons (3) represent agency regarding court involvement of the youth (4) link youth and family to appropriate pro-social activities that foster success and healthy development (7) respond to all complaints/concerns from T! FC parents in a timely supportive manner
7. Permanency Planning
To assure the development, implementation and monitoring of effective treatment services in transitional living for youth targeted the Treatment Coordinator will (1) confirm youth have completed an independent living assessment for life skills (2) submit assessment to the appropriate individuals for scoring (3) supervise any program aides that are assigned to the youth to assist in the implementation of the treatment and service plan (4) work in collaboration with other service providers to develop and implement intervention procedures in the homes working toward achievement of the permanency plan (5) coordinate all community resources pertaining to discharge placement

(6) arrange aftercare follow up if indicated
8. Professional Development
To assure continuous professional growth and development, the Treatment Coordinator will (1) actively participate in individual and/or group supervi! sion and staff meetings (2) attend and participate in all required and ! other professional trainings (3) independently seek out resources and learning opportunities to foster ongoing professional growth

Essential Qualifications

1. Education/Credentials/Licensure:
a. Bachelors Degree in human services (Psychology, Social Work, Counseling, Criminal Justice, Sociology or Behavioral Science); Masters Degree in human services preferred.
b. In West Virginia, Licensed Social Worker (LSW) or eligible for Temporary Licensed Social Worker (TLSW)
c. Valid drivers license and current insurance

2. Experience:
a) O ne year of supervised experience with children and/or families for Master’s level
b) Two years of supervised experience with children and/or families for Bachelor’s level .

3. Clearances. State Police/FBI clearance per state regulation; Child Abuse clearance
