Beckley WV is located amidst the picturesque Allegheny Mountains offering almost unlimited outdoor activities, such as hiking, white water rafting, skiing, golfing and cycling. We are conveniently located along I-77and I-64 and airport serviced by United Airlines with daily flights.

DUTIES:
The incumbent will be responsible to the Acute Care Service Line Outpatient Clinical Care Coordinator and will collaborate with interdisciplinary team members in the overall management of patients with acute and chronic renal disease. The Nephrology Case Manager will:

provide direct patient care in coordinating dialysis access for patients.

cooridinate inpatient dialysis access for acute patients as well as long term care residents.

provide coordination of care with community dialysis centers for outpatients.

provide coordination of care for renal transplant patients.
serve as the inpatient dialysis Contracting Officer Representative, overseeing all patients and contracted nursing staff performing dialysis.

QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:
U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Full unrestricted Licensure, Certification, or Registration required

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency

and

accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved schoo! l of professional nursing. The completion of coursework equivalent to a! nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)

Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

EDUCATION/EXPERIENCE REQUIREMENTS:
BSN with Nephrology Certification (CNN).

Demonstrates knowledge and clinical expertise in kidney disease beyond that required in a basic nursing program.

Ability to function independently in clinical nursing practice with a broad knowledge of patients along the continuum of kidney disease and all treatment modalities.

Exceptional interpersonal com! munication skills dealing effectively with internal and external customers.

Demonstrates self-direction, initiative, and the ability to care for patients with complex needs.

Evidence of leadership in improving patient care and initiative in quality improvement activities.

Demonstrates the ability to plan, coordinate and facilitate the continuity of care, inpatient and outpatient transition of the acute and chronic renal patient with complex problems.

Demonstrates the ability to utilize creativity and innovation to initiate performance improvement projects that improve patient care and clinic access/efficiency.

Preferred Experience:
Five years or more of increasingly complex nursing experience.

GRADE DETERMINATIONS:
Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional De! velopment, Collaboration and Scientific Inquiry. Each dimension of prac! tice has criteria that demonstrate essential core competencies for VHA Nursing Personnel.

The criteria are as follows:
Practice: practice, ethics and resource utilization;

Professional Development: performance, education/career development;

Collaboration; collaboration, collegiality;

Scientific Inquiry: quality of care, research.
